According to the relevant provisions of the Patent Administrative Law Enforcement Measures, if requesting the department responsible for managing patent work to handle patent infringement disputes, the following materials (in triplicate) shall be submitted:

1. The "Request for Handling Patent Infringement Disputes" shall include the following contents:

(1) The name or address of the requester, the name and position of the legal representative or main person in charge, and the name and address of the agent and the agency if authorized;

(2) The name or address of the requested party;

(3) The requested matters, facts, and reasons for processing;

(4) The request letter should be signed or stamped by the requester.

2. Proof of subject qualification, that is, individuals should submit their resident identity card or other valid identity documents, and units should submit a copy of their valid business license or other subject qualification certificate, as well as the identity certificate of their legal representative or main person in charge.

3. Power of Attorney: When entrusting someone as an agent, a power of attorney signed or stamped by the principal must be submitted.

4. Proof of valid patent rights, namely a copy of the patent registration book, or a patent certificate and receipt of payment of patent annual fees for the current year. Where a patent infringement dispute involves a patent for utility model or design, the administrative department for patent work may require the petitioner to issue a patent evaluation report made by the China National Intellectual Property Administration.

5. Relevant evidence materials that can preliminarily prove that the requested party has committed infringement of patent rights: a list of evidence materials should be submitted, classified and numbered one by one, and the source, proof object, and content should be briefly explained, signed and stamped, with the submission date indicated, and confirmed by the parties' signature. The requester shall provide copies of the request letter and relevant evidence according to the number of requested persons.
